Our Registered and Licensed Dietitians who consult in Long Term Care and Rehabilitation facilities will be involved in an initial comprehensive nutrition care assessment for each resident. This will be followed with monitoring of each resident on an as needed basis with input from facility staff and individual residents.

Particular emphasis of the assessment will focus on weekly and/or monthly resident weight, food and fluid intake, wounds, medications and relevant laboratory values.

Both the facility and residents will benefit from the consultant registered dietitian’s attendance and involvement in wound care and weight meetings.

As a part of the nutrition assessment process the Consultant Registered Dietitian will observe the preparation, service and acceptance of meals by individual residents.

Prior to service of a meal he/she will visually evaluate and taste the prepared food for all diets of regular and modified texture consistency. Our Consultant Registered Dietitians work with dietary and facility staff and will obtain information concerning residents as needed.

The Registered Dietitian works closely with dietary staff and the dietary manager to provide guidance in all aspects of food preparation and the kitchen.

The Consultant Registered Dietitian may also be involved in assisting with MDS preparation and care plan development.

As a part of the care plan process the Consultant Registered Dietitian meets with residents, their family and staff members.
Steps may be taken to improve quality of life for some residents.
